INDICATORS FOR A CONSULTATION WITH A PROFESSIONAL GERIATRIC CARE MANAGER

  • Family members have limited time, resources, or expertise; live at a distance; are confused or overwhelmed;  or need professional guidance regarding recommended services or housing options
  • A professional advocate is needed to deal with the complexities of the health care system
  • Responsibility for coordinating care requires a professional whose training is appropriate to that task
  • There is the possibility of needing a change of living situation, and a professional assessment would be helpful
  • Home management  and/or personal hygiene is being neglected
  • There is a new medical diagnosis or changes in functioning, mood  or cognition that might necessitate guidance, support, education, and/or monitoring
  • Safety concerns are present
  • Professional guidance is needed to help an elder remain at home with as much independence as possible
  • Family members have different opinions about the elder’s needs and would like an objective opinion
  • A plan is needed to identify the medical, physical, social, emotional, functional, housing, legal, or financial resources available for elders.
