About the GCM New England Organization
General information about who we are, and what we do as GCMs
Professional Geriatric Care Managers (GCMs) are specialists who assist older people and their families in designing plans for the elder which maximize independence and well being.
The National Association of Professional Geriatric Care Managers (GCM) defines and promotes the highest standards of private practice. Membership is open to qualified individuals either with a degree in a field of human services (social work, psychology, nursing, gerontology) or an equivalent background, who also are experienced in the delivery of services to the elderly and their families. Members of The New England Chapter of GCM must meet or exceed the standards of the National Association.
We can help...
- Conduct care-planning assessments to identify problems, eligibility for assistance, and need for services.
- Screen, arrange, and monitor in-home help or other services.
- Assist with moving a senior person into or from a retirement complex, care home, or nursing home.
- Act as a liaison to families at a distance, making sure things are going well and alerting families to problems.
- Review financial, legal, or medical issues and offer referrals to finance specialists to avoid future problems and conserve assets.
- Provide crisis intervention.
- Counsel and support (some provide psychotherapy)
- Explain the spectrum of available housing and care options, and facilitate transitions
- Educate the community
- Advocate for the elder consumer.
- Offer one time or continuing counseling and support.
- Provide services on a fee-for-service basis.

Mission:
GCM New England is an association whose purpose is to define and promote the highest standards of practice by professionals who are assisting older people and their families in the design and implementation of programs for the elder which maximize independence and well-being.
Members:
The association is made up of professionals representing themselves or a company, and are doing business in the field of geriatric care management. GCM members assist older persons and their families to cope with the challenges of aging. Most GCMs have master's degree level training in gerontology, social work, nursing or counseling.
Meetings:
GCM New England members meet monthly for professional development, organizational business and to network with others in the field.
